[iaufwg] Registering the FOREIGN extension name

This raises the question of how some of the current entries got there.
Having been through the FITS, IANA, and IETF process I find myself asking:
Is the XTENSION registration process described anywhere?
When is it just namespace reservation, akin to non-top-level MIME
types with IANA, and when is it something that deserves analysis
by committees?
My immediate impression is that any use of R and L and D and C might
just be namespace. S is clearly under the purview of the established
process for the standard.
Also, it might be nice for the XTENSION registration page to specify
more details about the existing entries:
DUMP who, where (in what context or reference), and when?
FILEMARK where and when?
VGROUP where and when?
FITS already has these features

I don't object, but in the absence of a defined process I'm not entirely
sure what it means to make the changes.
